It's easy to enroll in the TRICARE Supplement Plan. Just complete the RAUS Membership Application form. We will enroll you as a RAUS member and send you the complete package of information and Supplement Insurance Enrollment Form.

You cannot be turned down for coverage, although a pre-existing condition may initially limit the extent of your coverage. To download/view Pre-Existing Condition Plan Enhancements (Effective October 1, 2004) in  the Adobe Acrobat PDF format, click here.

After your completed Enrollment Form and first premium payment have been processed, you'll receive a certificate of insurance which you can examine for 30 days risk free. Return it for a full refund if you are not completely satisfied.

Step 1 - Download and print the RAUS Membership Application to join RAUS (if you are not already a member).

Step 2 - Download and print one of the enrollment forms:

• Group TRICARE Standard/Extra Supplement Plan Enrollment Form - For residents of all states except NY
• Group TRICARE Standard/Extra Supplement Plan Enrollment Form - For NY State residents

Step 3 - Select one of the following plans: optional plans:

• High Option II Retiree Plan
• Active Duty Family II Plan

Step 4 - Determine your quarterly premiums based on your selected plan and your present age. Don't forget to include coverage for a spouse and/or children.

Step 5 - Complete and mail all forms along with your payment for membership dues and premium amount to:
