It's easy to enroll in the TRICARE Prime Supplement Plan. Just complete the RAUS Membership Application form. We will enroll you as a RAUS member and send you the complete package of information and Supplement Insurance Enrollment Form.

After your completed Enrollment Form and first premium payment have been processed, you'll receive a certificate of insurance which you can examine for 30 days risk free. Return it for a full refund if you are not completely satisfied.

Step 1 - Download and print the following individual forms:

• RAUS Membership Application (required if you are not already a member)

• RAUS TRICARE Prime Supplement Plan Enrollment Form - For residents of all states except NY

• RAUS TRICARE Prime Supplement Plan Enrollment Form - For NY State residents

Step 2 - Print your name and address clearly on the enrollment form.

Step 3 - Determine your premium based on your selected plan (Plan A or B).

Step 4 - Sign and date the Enrollment Form.

Step 5 - Calculate your quarterly premium from the rate schedule. Premiums shown are PER Person.

Step 6 - Make your premium check payable to "RAUS Group Health Program" and mail with your discounted initial RAUS membership dues payable to RAUS to:
